Tanya Sobaski, DNP, MSN, BSN, RN, NP-C is a Doctor of Nursing Practice and Family Nurse Practitioner with nearly 30 years of healthcare experience. Her background includes acute care nursing, nursing education, family practice, and urgent care. Throughout her career, she has been dedicated to process improvement, patient advocacy, and helping individuals achieve optimal health.
After witnessing the increasing prevalence of chronic disease and the limitations of symptom-focused care, Tanya pursued advanced training in functional and nutritional medicine. She is a graduate of the American Academy of Functional Health and is certified as both a Functional Weight Loss Practitioner and a Functional Health and Nutritional Medicine Practitioner.
Her passion for helping people identify and address the root causes of illness led her to leave Corewell Health and establish her own functional health consulting practice. Tanya works with clients experiencing chronic health conditions, autoimmune disorders, hormone imbalances, digestive concerns, weight management challenges, and other complex health issues.
Through a personalized approach that incorporates nutrition, lifestyle modifications, and targeted supplementation, Tanya helps clients restore balance, improve wellness, and achieve lasting health. Her virtual practice offers convenient, individualized support for those seeking a holistic, evidence-informed path to better health.
Why Functional Medicine?
Functional Medicine is a personalized, patient-centered approach to healthcare that focuses on identifying and addressing the root causes of illness rather than simply treating symptoms. Instead of asking, "What medication can be used for this condition?" Functional Medicine asks, "Why is this happening in the first place?"
Many chronic health conditions, including autoimmune diseases, hormone imbalances, digestive disorders, fatigue, weight gain, and inflammation, can be influenced by factors such as nutrition, lifestyle, stress, environmental exposures, genetics, and gut health. By looking at the whole person and understanding how these factors interact, Functional Medicine seeks to uncover the underlying imbalances that may be contributing to disease.
Traditional medicine plays a vital role in diagnosing and treating acute illnesses and emergencies. Functional Medicine complements that care by taking a deeper look at what may be driving ongoing symptoms and chronic conditions. Through comprehensive evaluation and personalized treatment plans, the goal is to restore balance, support the body's natural healing processes, and help individuals achieve long-term health and wellness.
At its core, Functional Medicine recognizes that every person is unique. By listening to your story, understanding your health history, and addressing the factors that influence your well-being, we can develop a tailored plan designed to help you feel your best and live a healthier, more vibrant life.
How is Functional Medicine different from other forms of alternative medicine?
Both conventional Western medicine and Functional Medicine play important roles in healthcare, but they differ in their approach to patient care.
Conventional medicine excels at diagnosing and treating acute illnesses, infections, injuries, and medical emergencies. It often focuses on identifying a disease and managing symptoms through medications, procedures, or surgery when necessary.
Functional Medicine takes a more comprehensive, patient-centered approach. Rather than asking only, "What disease does this person have?" it also asks, "Why did this disease develop?" and "What factors may be contributing to it?" The goal is to identify and address the underlying causes of health concerns rather than simply managing symptoms.
One of the most significant differences is the time spent with patients. Functional Medicine practitioners typically devote more time to understanding a patient's complete health history, lifestyle, nutrition, stress levels, environmental exposures, and personal health goals. This allows providers to truly listen to each patient's story and gain a deeper understanding of the factors influencing their health.
Functional Medicine recognizes that factors such as nutrition, lifestyle habits, stress, sleep quality, gut health, environmental exposures, genetics, hormone balance, and inflammation can all influence health and disease. By evaluating these interconnected factors, practitioners develop personalized care plans designed to support the body's natural ability to heal and function optimally.
Key differences include:
-
Root-cause focus: Identifying and addressing the underlying factors contributing to illness rather than solely treating symptoms.
-
Personalized care: Recognizing that each person is unique and may require a different treatment approach, even if they share the same diagnosis.
-
Whole-person perspective: Considering physical, emotional, mental, lifestyle, and environmental influences on health.
-
More time and deeper listening: Creating space for patients to be heard, understood, and actively involved in their healthcare journey.
-
Partnership approach: Building a collaborative relationship between provider and patient to achieve health goals.
-
Lifestyle-based interventions: Emphasizing nutrition, exercise, stress management, sleep optimization, and targeted supplementation as foundational tools for improving health.
Functional Medicine is particularly helpful for individuals struggling with chronic health concerns, autoimmune conditions, hormone imbalances, digestive disorders, fatigue, inflammation, and metabolic health challenges. Rather than replacing conventional medicine, it complements it by providing a more personalized and comprehensive approach to achieving long-term wellness.
At its core, Functional Medicine empowers patients with the knowledge, tools, and support they need to take an active role in their health and create lasting change.

Tailored Functional Wellness Review:
I became a patient of Tanya Sobaski’s in November of 2025, and I have been very pleased with my results. Tanya is professional, thorough, and truly cares about her patient’s wellbeing. She listened carefully, answered all my questions, and made me feel at ease throughout the entire process. Thanks to Tanya’s tailored wellness plan, I have seen tremendous improvements in the areas of my mental health, reduced inflammation, lowered blood pressure, and lowered cholesterol levels. I would recommend Tailored Functional Wellness to any prospective patient that is looking for results.
Respectfully, Jeff K.

Thank you for helping me with my periods. They were almost debilitating, so heavy and bad cramps. The plan you developed for me made such a difference!
I want you to know that I am starting to feel better like my old self again. The hot flashes, irritability, moodiness, lack of energy, and joint pain have improved. I gave a friend your number, she's been feeling like I was and she is so interested in learning more about how you can help her too!
Thank you,
Erica M.
